**Job Description**

As a Support Worker - Extra Care, you will be responsible for providing person-centered care and support services to residents living in our Extra Care schemes.

About the Scheme:

Our Extra Care schemes offer a unique blend of independence and support, enabling residents to live safely and confidently in their own apartments.

Your Key Responsibilities:

  • Support residents with daily living tasks, such as personal care, medication management, and domestic duties.
  • Develop and implement personalized care plans in collaboration with residents, families, and healthcare professionals.
  • Monitor and report changes in resident condition, ensuring timely interventions and referrals.
  • Collaborate with colleagues to ensure seamless delivery of care services.

Requirements:

  • Experience working in adult social care or a related field.
  • Excellent communication and interpersonal skills.
  • Able to work flexibly and respond to changing situations.
  • Possess a calm and empathetic demeanor.
