Resettlement Officer

This role involves supporting adult males who are transitioning from custody or community settings to independent living.
You will manage a caseload of participants and develop individualised plans to address their accommodation needs and facilitate successful resettlement.
Key Responsibilities:- Manage a caseload of participants producing personalised accommodation plans
- Carry out initial assessments considering risk situation and need
- Record progress and outcomes timely and accurately using case management system
- Maintain contact with participants identifying any emerging risks and concerns
- A competitive salary up to £28,000 per annum depending on experience
