The Role
We are seeking a highly motivated and experienced Care Coordinator to join our team. As a Care Coordinator, you will be responsible for sourcing and brokering support services for vulnerable children and young people. You will work closely with our clients to understand their needs and provide tailored solutions to meet their requirements.
Key Responsibilities
- Sourcing and brokering support services for vulnerable children and young people
- Working closely with clients to understand their needs and provide tailored solutions
- Developing and maintaining relationships with service providers
- Ensuring compliance with relevant legislation and regulations
- Providing excellent customer service and support to clients and service providers
Requirements
- Proven experience in a similar role or in a related field
- Strong knowledge of the children and young people's health and social care sector
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work occasional early evening shifts
