The Role

We are seeking a highly motivated and experienced Care Coordinator to join our team. As a Care Coordinator, you will be responsible for sourcing and brokering support services for vulnerable children and young people. You will work closely with our clients to understand their needs and provide tailored solutions to meet their requirements.

Key Responsibilities

  • Sourcing and brokering support services for vulnerable children and young people

  • Developing and maintaining relationships with service providers

Requirements
