Care Coordinator Job Description

We are seeking a skilled Care Coordinator to join our team at Social Care and Education Jobs in Leeds, West Yorkshire. This is a full-time, permanent position with on-call hours.

Job Summary:

The successful candidate will play a pivotal role in scheduling and managing care rotas for our dedicated team of homecare workers. As a Care Coordinator, you will be responsible for planning and organizing rotas to ensure continuity and quality of care, managing day-to-day scheduling changes, and liaising with clients and their families to arrange and confirm care schedules.

Key Responsibilities:

  • Plan and organize care rotas to ensure continuity and quality of care.
  • Manage day-to-day scheduling changes and ensure all shifts are covered.
  • Liaise with clients and their families to arrange and confirm care schedules.
  • Monitor care plans, log changes, and update our Staff Plan system.
  • Provide support and guidance to the care team, respond to queries, and handle emergencies when on call.
  • Ensure all regulatory and compliance standards are met in every care plan and visit.

Requirements:

  • A minimum of 2 years' experience in homecare.
  • Proven experience in care coordination and rota planning.
  • A car driver's license is essential.

Estimated Salary: £25,000 - £30,000 per annum, depending on experience.
