Job Description

We are seeking a skilled Community Care Specialist to join our team. In this role, you will work closely with individuals who require 1-1 support in the community, providing assistance with daily tasks and promoting their independence.

You will be part of a supportive team that prioritizes empathy, compassion, and sensitivity towards others. Our staff receive specialist training in autism, mental health, and learning disabilities, ensuring they are equipped to deliver high-quality care.

The ideal candidate will possess patience, empathy, and a strong work ethic, with a commitment to delivering exceptional care and support.

Key Responsibilities
  • Provide 1-1 support to individuals in the community, assisting with daily tasks and promoting independence
  • Work collaboratively with the support team to ensure seamless care delivery
  • Attend regular team meetings and contribute to discussions on care planning and service development
  • Maintain accurate records of care delivered and progress made
