Job Description

Care Coordinator Job Vacancy in Leeds, West Yorkshire.

Estimated salary: £25,000 - £30,000 per annum.

We are a reputable family-owned homecare provider delivering quality adult social care across Leeds, Bradford, and Wakefield.

Serving both private clients and local authority contracts, our services support individuals to live independently at home.

We utilize advanced scheduling software, Staff Plan, for efficient care planning and seamless client experiences.

  • This role is pivotal in managing care rotas for our dedicated team of homecare workers.
  • The successful candidate will oversee rota planning, ensuring continuity and quality of care.
  • Liaising with clients and their families to arrange and confirm care schedules.
  • Monitoring care plans, logging changes, and updating our system.
  • Providing support and guidance to the care team, responding to queries, and handling emergencies.

A minimum of 2 years' experience in homecare is required.

  • Proven experience in care coordination and rota planning is essential.
  • A car driver is required for this position.
  • Proficiency with computer systems and scheduling software is necessary.
