Job Summary: We are seeking a skilled and experienced Nurse Assessor to join our team at CES Locums. As a Senior Clinical Lead for Learning Disability Assessments, you will be responsible for conducting robust and transparent assessments for NHS Continuing Healthcare/NHS-funded Nursing Care.

Key Responsibilities:

  • Manage a complex clinical caseload, including assessments, reviews, and implementation of Personal Health Budgets and ongoing Case Management of Patients funded by Continuing Healthcare (CHC), Mental Health & Learning Disabilities.
  • Ensure assessments are conducted in a multi-agency setting (where required) for NHS Continuing Healthcare/NHS-funded Nursing Care.
  • Assess new applications for client's eligibility against the National Framework for Continuing Healthcare and Funded Nursing Care.
  • Participate in Safeguarding of Vulnerable Adults process related to individuals in Care Homes with Nursing.
  • Undertake robust quality Nursing Assessments and Reviews.
