Main Duties
- Conduct thorough assessments of referrals/admissions to the service, identifying areas of risk and developing effective treatment plans.
- Manage risk, including vulnerability, suicide risk, risks to health and safety, and risk of harm to others, ensuring the highest level of patient safety.
- Collaborate with the Care Home Liaison Team to develop and implement effective care pathways.
