Job Description: As a Home Based Occupational Therapist, you will work with Social Workers and Assessors to promote wellbeing and independence by completing assessments, support plans, and utilizing specialist equipment. Your role will involve understanding local people's care and support needs, identifying options, and ensuring they are connected to the necessary services and community assets.

Required Skills and Qualifications: To succeed in this role, you will need experience working in Social Care and/or Health community settings, effective communication skills, and a demonstrable working knowledge of Health and Social Care Policy, housing, and legislation affecting OT practice.
