About the Role

We are seeking a highly skilled and experienced Consultant Advanced Clinical Practitioner to join our team at Kent Community Health NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing expert clinical care and leadership to our patients and staff.

Key Responsibilities
  • Provide advanced clinical expertise in frailty care, including assessment, diagnosis, and management of complex patients.
  • Lead and develop clinical services, ensuring high-quality patient care and outcomes.
  • Collaborate with multidisciplinary teams to develop and implement evidence-based practice.
  • Contribute to education and training programs, ensuring staff have the necessary skills and knowledge to provide excellent patient care.
  • Participate in research and quality improvement initiatives, driving innovation and excellence in frailty care.
Requirements
  • First-level registration with the NMC, HPCP, or GPHC.
  • Minimum of 5 years post-registration experience, with experience at Band 8a or above.
  • Master's level or higher degree in a relevant field.
  • Teaching and research qualifications.
  • Leadership and non-medical prescribing experience.
Desirable Qualifications and Experience
  • Psychology or counseling experience and training/education.
  • Extensive experience in report writing, planning, and implementation of educational programs.
  • Advanced IT skills and experience with evidence of audit, research, and innovation applied in practice.

Working Arrangements

This is a full-time position, working 37.5 hours per week. The successful candidate will be required to work flexibly, including weekends, to meet the needs of the service.
