About the Role

We are seeking a highly skilled and experienced Consultant Advanced Clinical Practitioner to join our team at Kent Community Health NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to our patients with frailty.

Key Responsibilities
  • Lead the development and implementation of clinical strategies to improve patient outcomes and experiences
  • Provide expert clinical leadership and guidance to colleagues, ensuring the delivery of high-quality care
  • Develop and deliver education and training programs to enhance the skills and knowledge of staff
  • Contribute to the development and evaluation of clinical policies and procedures
  • Collaborate with stakeholders to identify and address gaps in care and services
Requirements
  • First-level registration with the NMC, HPCP, or GPHC
  • Minimum of 5 years post-registration experience, with experience at Band 8a or above
  • Master's level or higher degree in a relevant field
  • Teaching and research qualifications
  • Leadership and non-medical prescribing experience
Desirable Qualifications and Experience
  • Psychology or counseling experience and training/education
  • Extensive experience in the care and treatment of people with frailty, including long-term conditions and dementia care
  • Advanced skills in history taking and physical assessment of frail people
  • Independent prescribing experience
