About the Role

We are seeking a highly skilled and experienced Consultant Advanced Clinical Practitioner to join our team at Kent Community Health NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to our patients with frailty.

Key Responsibilities
  • Lead the development and implementation of clinical strategies to improve patient outcomes and experiences
  • Provide expert clinical leadership and guidance to colleagues, ensuring the highest standards of care are maintained
  • Contribute to the development and delivery of education and training programs for staff and students
  • Collaborate with stakeholders to develop and implement service improvements and innovations
  • Manage and coordinate the work of the multidisciplinary team to ensure seamless care delivery
Requirements
  • First-level registration with the relevant professional body (NMC, HPCP, or GPHC)
  • Minimum of 5 years post-registration experience, with experience at Band 8a or above
  • Master's level or higher degree in a relevant field
  • Teaching and research qualifications
  • Leadership and non-medical prescribing experience
Desirable Qualifications and Experience
  • Psychology or counseling experience and training/education
  • Extensive experience in the care and treatment of people with frailty, including long-term conditions and dementia care
  • Advanced skills in history taking and physical assessment of frail people
  • Independent prescribing experience
