About the Role

We are seeking a highly skilled and experienced Consultant Advanced Clinical Practitioner to join our West Kent Home Treatment Service. As a clinical expert in Frailty, you will lead the Home Treatment Service alongside the SAS Doctors West Kent.

Key Responsibilities
  • Provide advanced and expert-level clinical practice within the field of Frailty, ensuring clinical excellence.
  • Lead and consult with patients, carers, and colleagues, providing expert professional advice.
  • Develop and provide education and training to other staff and trainees.
  • Contribute to service development, evaluation, research, and audit.
About Us

Kent Community Health NHS Foundation Trust is a community team made up of various staff, including Specialist Doctors, Speciality Doctors, Advanced Clinical Practitioners, and Health Care Assistants. We have excellent links with Primary Care, Community Teams, Hospice, SECAMB Trust, and Acute Trust Teams.

Requirements
  • First-level registration with NMC, HPCP, or GPHC.
  • Minimum of 5 years post-registration experience, with experience at Band 8a or above.
  • Master's level or higher degree.
  • Teaching qualification.
  • Research qualification.
  • Leadership.
  • Non-medical prescribing.
Desirable Qualifications and Experience
  • Psychology or counselling experience and training/education.
  • Extensive experience of presenting reports, education, and information to public forums.
