About the Role

We are seeking a highly skilled and experienced Consultant Advanced Clinical Practitioner to join our West Kent Home Treatment Service. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to our patients.

Key Responsibilities
  • Lead the Home Treatment Service alongside the SAS Doctors West Kent, ensuring seamless collaboration and effective care delivery.
  • Develop and implement strategies to improve patient outcomes, working closely with colleagues and partners to achieve this goal.
  • Provide expert clinical guidance and support to patients, carers, and colleagues, ensuring excellence in practice.
  • Contribute to the development and delivery of education and training programs, enhancing the skills and knowledge of our team.
  • Participate in national and international research, contributing to the development of a centre of excellence in care and treatment for older people.
Requirements
  • First-level registration with NMC, HPCP, or GPHC.
  • Minimum of 5 years post-registration experience, with experience at band 8a or above.
  • Master's level or higher degree.
  • Teaching and research qualifications.
  • Leadership and non-medical prescribing experience.
Desirable Qualifications and Experience
  • Psychology or counselling experience and training/education.
  • Extensive clinical experience in the care and treatment of people with frailty, including long-term conditions and dementia care experience.
