We are seeking a highly skilled and experienced Consultant Advanced Clinical Practitioner to join our West Kent Home Treatment Service. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to our patients.
Key Responsibilities- Lead the Home Treatment Service alongside the SAS Doctors West Kent, ensuring seamless collaboration and effective care delivery.
- Develop and implement strategies to improve patient outcomes, working closely with colleagues and partners to achieve this goal.
- Provide expert clinical guidance and support to patients, carers, and colleagues, ensuring excellence in practice.
- Contribute to the development and delivery of education and training programs, enhancing the skills and knowledge of our team.
- Participate in national and international research, contributing to the development of a centre of excellence in care and treatment for older people.
- First-level registration with NMC, HPCP, or GPHC.
- Minimum of 5 years post-registration experience, with experience at band 8a or above.
- Master's level or higher degree.
- Teaching and research qualifications.
- Leadership and non-medical prescribing experience.
- Psychology or counselling experience and training/education.
- Extensive clinical experience in the care and treatment of people with frailty, including long-term conditions and dementia care experience.
We are a community team made up of a variety of staff, including Specialist Doctors, Speciality Doctors, Advanced Clinical Practitioners, and Health Care Assistants. We have excellent links with Primary Care, Community Teams, Hospice, SECAMB Trust, and Acute Trust Teams.
