About the Role

We are seeking a highly skilled and experienced Consultant Advanced Clinical Practitioner to join our team at Kent Community Health NHS Foundation Trust. As a key member of our Frailty Service, you will play a vital role in delivering high-quality patient care and contributing to the development of our services.

Key Responsibilities
  • Lead the Frailty Service, working closely with our multidisciplinary team to develop and implement effective care pathways.
  • Provide expert clinical leadership, ensuring that our patients receive the highest standard of care.
  • Develop and deliver education and training programs for our staff, promoting excellence in clinical practice.
  • Contribute to the development of our service, including strategy, policy, and procedure development.
  • Collaborate with our partners, including primary care, community teams, and acute trusts, to ensure seamless care transitions.
Requirements
  • First-level registration with the NMC, HPCP, or GPHC.
  • Minimum of 5 years post-registration experience, with experience at Band 8a or above.
  • Master's level or higher degree in a relevant field.
  • Teaching and research qualifications.
  • Leadership and non-medical prescribing experience.
Desirable Qualifications and Experience
  • Psychology or counselling experience and training/education.
  • Extensive experience in the care and treatment of people with frailty, including long-term conditions and dementia care.
