As a key member of our care management team, you will be responsible for managing a team of care professionals, developing and implementing care plans, and maintaining high standards of care.

Job Description

This is an exciting opportunity to join a dynamic and forward-thinking organization that is committed to delivering exceptional care services. You will work closely with our senior leadership team to develop and implement care strategies, manage staff performance, and ensure compliance with relevant laws, regulations, and policies.

About the Role

We are offering a competitive salary of £38,000 - £48,000 per annum, depending on experience, plus a range of benefits including pension scheme, life insurance, and 25 days annual leave.

Responsibilities
  • Develop and implement effective care plans to meet the needs of residents
  • Manage a team of care professionals to deliver high-quality care services
  • Ensure compliance with relevant laws, regulations, and policies
  • Monitor and evaluate the effectiveness of care services and make recommendations for improvement
  • Participate in quality assurance activities and contribute to the development of quality improvement initiatives
Requirements

To be successful in this role, you will need:

  • A degree in social work, nursing, or a related field
  • At least 2 years' experience in adult care management
  • Proven track record of delivering outstanding results in a similar role
  • Excellent communication and leadership skills
  • Ability to work effectively in a fast-paced environment
