About the Role

We are seeking a skilled Nurse Specialist to join our team at Danone. As a key member of our community care team, you will provide high-quality care and support to patients in their own homes.

Key Responsibilities

  • Develop and deliver comprehensive training to patients, carers, and healthcare professionals to support safe discharges from hospital and ongoing care at home.
  • Complete regular patient reviews and provide ongoing support with questions, issues, and troubleshooting requests.

About You

We are looking for a motivated and compassionate individual with a strong background in nursing and experience working with patients in their own homes. You will have excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
