About the Role

We are seeking a skilled and compassionate Nurse Specialist to join our team at Danone. As a key member of our community care team, you will provide high-quality care and support to patients in their own homes, working closely with healthcare professionals and carers to ensure seamless care delivery.

Key Responsibilities

  • Develop and deliver comprehensive training programs to patients, carers, and healthcare professionals to support safe discharges from hospital and ongoing care at home.
  • Conduct regular patient reviews and provide ongoing support and troubleshooting as needed.
  • Build strong relationships with patients, carers, and healthcare professionals to ensure effective care coordination and delivery.

About You

We are looking for a highly motivated and organized individual with excellent communication and interpersonal skills. You will have a strong background in nursing, with experience in community care and a passion for delivering high-quality patient care. You will be able to work independently and as part of a team, with a strong focus on patient-centered care and outcomes.
