An exciting opportunity has arisen to work as a Community Health Nurse and join the Aylesbury Adult Community Healthcare Team.
Job DescriptionThe post is to provide nursing interventions to patients in their home environment, contributing to processes for service and quality improvement. Key responsibilities include assessing the physical and psycho-social needs of patients, delivering therapeutic interventions based on best available evidence to improve health outcomes, and managing own case load.
Requirements- Demonstrate profession specific clinical assessment, planning, delivery and evaluation skills.
- Knowledge of modernisation agenda within the NHS and social care.
- Report writing skills.
We offer a competitive salary of £30,000 - £50,000 per annum, plus benefits including generous annual leave entitlement, pension, and access to NHS discount schemes.
