About the Role

We are seeking a highly skilled and experienced Community Mental Health Nurse to join our team at the South London and Maudsley NHS Foundation Trust. As a Care Coordinator, you will work closely with service users, carers, and other healthcare professionals to provide high-quality care and support.

Key Responsibilities
  • Formulate and implement holistic care plans with service users and carers
  • Work collaboratively with the multidisciplinary team to provide comprehensive care
  • Conduct dynamic risk assessments and develop strategies to mitigate risks
  • Provide education and support to service users and carers on mental health issues
  • Participate in supervision and peer review to ensure high-quality care
Requirements
  • Registered Mental Health Nurse with significant relevant clinical experience
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Knowledge of clinical governance and audit principles
  • Experience in care coordination and risk assessment
