Key Responsibilities:- Manage a caseload of clients, undertaking initial engagement, risk assessment, and providing support in accessing mental health services.
- Facilitate evidence-based interventions to reduce reoffending, utilizing the trauma recovery model and restorative approaches.
- Ensure seamless access to mental health services, promoting prevention and recovery.
- Identify and engage with a network of agencies and services to support offenders in the community, addressing individual needs for education, training, employment, accommodation, and health support.
- Coordinate multi-agency meetings to manage risk and identify lead professionals for each service user.
