Learning Disability Case Manager

We are looking for an experienced and skilled Transforming Care Lead to join our team at North Staffordshire Combined Healthcare NHS Trust.
The successful candidate will work closely with the Learning Disability Transforming Care Team to support the delivery of Transforming Care across all Trust Divisions.
This role involves participating in assessment, planning, and implementation of proactive care and case management for individuals with learning disabilities and autism.
As a member of the Learning Disability Transforming Care Team, you will be working towards improving the health and wellbeing of local people through high-quality care.
Main ObjectivesThe purpose of this role is to support the commissioning of safe and effective services for people with a learning disability, supporting discharge from hospital and oversight of commissioned community packages of care.
This involves participating in assessment, planning, and implementation of pro-active care and case management for the population of Staffordshire and Stoke-on-Trent with a Learning Disability and/or autism.
The post holder will be a member of the Learning Disability Transforming Care Team within the Specialist Directorate of North Staffordshire Combined Healthcare NHS Trust.
Key Skills- Experience of managing teams and projects, with a proven track record of delivering results.
- Strong understanding of Transforming Care principles and policies.
Estimated salary: £60,000 per annum.
