Job Description: Learning Disability Support Specialist
This is an exciting opportunity to join our team as a Learning Disability Support Specialist, working with individuals with complex needs and their families to provide personalized support and guidance.
You will be responsible for developing and implementing care plans, coordinating community-based care packages, and working closely with multidisciplinary teams to ensure seamless care transitions.
Main Responsibilities- To assess and develop care plans that meet the unique needs of individuals with learning disabilities.
- To coordinate community-based care packages, ensuring that services are delivered efficiently and effectively.
- To provide expert guidance and support to individuals, families, and caregivers.

Requirements and Qualifications- Qualified nurse with a strong background in learning disability/mental health care.

Estimated Salary: £58,000 per annum, commensurate with experience.
