Job Title: Hospital Care Specialist - Kent and Canterbury

We are seeking a skilled Hospital Care Specialist to join our team at East Kent Hospitals University NHS Foundation Trust. As a Hospital Care Specialist, you will be responsible for delivering high-quality care to patients in their own homes.

Job Description:

As a Hospital Care Specialist, you will be responsible for:

  • Managing a caseload of patients in their own homes
  • Delivering holistic care, working collaboratively with multidisciplinary teams
  • Developing and implementing care plans that meet the unique needs of each patient
  • Monitoring patient progress, identifying areas for improvement, and implementing changes as necessary

Requirements:

To be successful in this role, you will need:

  • A degree in Nursing or a related field
  • Post-registration experience in a hospital setting
  • Strong leadership and communication skills
  • Ability to work independently and as part of a team

Salary: £55,000 - £65,000 per annum
