Stroke Rehabilitation Nurse

We are seeking a highly skilled and dedicated Stroke Rehabilitation Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a key member of our interdisciplinary rehabilitation unit, you will play a vital role in providing high-quality care to patients who have suffered a stroke.
Key Responsibilities- Provide nursing services to patients with complex needs, including assessment, diagnosis, and treatment of stroke-related conditions.
- Manage a defined caseload, working independently and as part of a team to achieve patient-centered goals.
- Participate in multidisciplinary rehabilitation goals, ensuring a functional approach to patient care.
- Provide clinical interventions to meet the holistic needs of patients, including personal care, wound management, and medicines management.
- Make informed clinical decisions, using evidence-based practice and outcome measures to deliver effective treatment.
- Work within defined protocols and policies for stroke care, including NICE clinical guidelines and the National Stroke Strategy.
- Be professionally and legally accountable for all aspects of your work, ensuring that it meets professional standards and clinical guidelines.
- Adapt your practice to meet individual patient and family circumstances, including complex clinical conditions.
- Monitor patients' general medical condition and respond appropriately.
- Ensure that patients, families, and carers are involved in the planning and prioritization of their stroke rehabilitation care plans.
- Work collaboratively with other services, including social services and the volunteer sector.
- Current NMC registration level 1.
- Varied clinical experience, including assessment and general rehabilitation.
- Previous experience of working as part of a multidisciplinary team.
- Knowledge of current nursing practice and issues.
- Ability to work independently and as part of a team.
- IT knowledge and ability to communicate effectively with colleagues, other disciplines, patients, relatives, and carers.
- Ability to deal with distressing and emotionally difficult situations.
- Knowledge of the role of other members of the multidisciplinary team.
- Ability to prioritize workload, organize, and delegate effectively.
- Observation and reporting skills.
- Evidence of own development through PDP.
- Able to support junior members of staff.
This is a full-time position, working over a 7-day rota, covering 24 hours a day in an inpatient setting. You will be required to work a rotation of shifts, including early, long days, and night shifts, covering weekdays, weekends, and bank holidays.
