We are seeking a skilled and compassionate Clinical Support Worker to join our team within the Stroke Pathway at Norfolk Community Health and Care NHS Trust. Key Responsibilities- Accurately and promptly carry out instructions from Registered Nurses to ensure seamless care delivery.
- Perform duties as specified in the trust's Approved Core Task List for Clinical Support Workers, working within assessed competence.
- Report any patient concerns or observed changes in clinical condition to registered staff.
- Support patients' emotional well-being by engaging in positive conversations and encouraging participation in ward-based activities.
- Accurately document all care delivered to patients, maintaining confidentiality at all times.
- Respond appropriately to clinical emergencies to ensure patients receive timely care.
- Adhere to trust policies, procedures, and nursing guidelines when undertaking activities.
- Make patients feel welcome and participate in the admission process under the supervision of registered staff.
- Maintain a clean and safe environment in the clinical area.
- Adhere to trust infection control procedures.
- Support relatives under the guidance of a Registered Nurse.
- Assist with the discharge process for patients directed by Registered Nurses or the Multidisciplinary Team.
- Welcome patients, visitors, and relatives to the clinical area, providing assistance, support, and information where required.
- Answer the telephone in a polite and courteous manner, responding appropriately within remit and passing on enquiries to the Registered Nurse.
- Communicate effectively within the nursing team, multidisciplinary team, and other departments to ensure patient care is well-planned and coordinated.
- Contribute to the handover of patient care within the team under the direct supervision of a Registered Nurse, maintaining confidentiality at all times.
- Report and record incidents, accidents, and complaints using the trust's systems (Datix) to the senior nurse on duty.
- Use trust systems such as System One and ICE as required by the clinical area.
- Ensure the safe keeping of patient property in accordance with trust policy.
- Promote a professional, welcoming, and uncluttered environment, keeping all storage areas safe and clean and tidy.
- Share responsibility in the care and maintenance of all equipment in the clinical area.
- Dispose of waste and soiled linen in line with trust policy.
- Participate in audit under the direction of the Registered Nurse.
- Clean and check beds, lockers, oxygen, and suction equipment in the clinical area following patient discharge and in preparation for admission.
- Ensure up-to-date completion of mandatory training sessions, including safeguarding and safeguarding supervision.
- Have completed the Care Certificate or relevant experience in a care setting.
- Be flexible in relation to patient requirements.
- Continue personal development in relation to identified objectives.
- Supervise and support new staff, working with the local induction program.
- Maintain up-to-date knowledge of changes in policies and procedures relevant to the post.
- Participate in training and supervision sessions relevant to the clinical area, including clinical competences where appropriate.
- Ensure attendance at an annual appraisal is maintained and documented.
