About the Role

We are seeking a highly skilled and dedicated Stroke Rehabilitation Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a key member of our interdisciplinary rehabilitation unit, you will play a vital role in providing high-quality nursing care to patients with complex needs following a stroke.

Key Responsibilities
  • Provide nursing care to patients with stroke-related conditions, including assessment, diagnosis, and treatment.
  • Manage a defined caseload with support from senior staff members, ensuring effective communication and collaboration with the multidisciplinary team.
  • Participate in patients' multi-disciplinary rehabilitation goals, focusing on functional approaches to rehabilitation and addressing personal care, wound management, and other holistic needs.
  • Make clinical decisions based on evidence-based practice and outcome measures, seeking advice and support as needed.
  • Work within defined protocols and policies for stroke care, including NICE clinical guidelines and the National Stroke Strategy.
  • Be professionally and legally accountable for all aspects of your work, ensuring adherence to professional standards and clinical guidelines.
  • Adapt your practice to meet individual patient and family circumstances, including complex conditions related to communication, cognitive, emotional, and physical impairments.
  • Monitor patients' general medical condition and respond appropriately.
  • Ensure patients, families, and carers are involved in planning and prioritizing rehabilitation care plans.
  • Work collaboratively with other services, such as social services and the volunteer sector.
Requirements
  • Current NMC registration level 1.
  • Varied clinical experience, including assessment and working within a multidisciplinary team.
  • General rehabilitation experience.
  • Ability to work independently and as part of a team, with excellent communication and organizational skills.
  • Knowledge of current nursing practice and issues, including stroke-specific nursing.
  • Ability to prioritize workload, organize, and delegate effectively.
  • Observation and reporting skills, with evidence of own development through PDP.
  • Able to support junior members of staff.
  • Ability to work over a 7-day rota, including rotation of shifts.
