The Primary Purpose: As a Care and Housing Liaison Officer, your primary purpose will be to support Service Users in making positive change and gaining life skills to meet their goal of moving on to independent living. You will do this by working across our Housing Related Support Services and collaborating with Service Users to create individual support plans.

Your Key Tasks:

  1. Support Service Users to gain life skills and meet their goal to move on to independent living.
  2. Help Service Users achieve their ambitions in areas such as health, budgeting, training, and employment.
  3. Ensure seamless service delivery by collaborating with other teams.
  4. Maintain accurate records and contribute to quality assurance processes.

Requirements:

  • Degree in Social Work, Psychology, or related field (or equivalent experience).
  • Full driving license and access to a vehicle for regular travel.
  • Excellent communication and interpersonal skills.
  • Able to work independently and as part of a team.
