Job Role

This Support Worker role is critical in guiding young people aged 16-18 through the transition from care to independent living. You will work closely with the young person, their social worker, and other allied professionals to deliver bespoke support based on their unique needs and goals.

Key Responsibilities
  • Care Planning: Develop and implement personalized care plans in collaboration with the young person, their social worker, and other relevant parties.
  • Collaboration and Liaison: Liaise with social workers, mental health professionals, and other relevant parties to ensure a coordinated approach to the young person's care.
  • Safeguarding: Reporting safeguarding concerns to senior staff within the organization and following support plans to help young people achieve their goals.
