Discharge Process Coordinator Position Available

We are looking for an experienced professional to join our team as a Discharge Process Coordinator. This exciting opportunity will allow you to work closely with our intermediate care services to ensure the smooth running of discharge processes and improve patient outcomes.

About the Role
  • Provide advice and support to the wider members of the intermediate care services and multi-disciplinary teams.
  • Proactively work alongside the intermediate care services to focus on the internal patient flow journey.
  • Maintain clear and precise documentation in patient records.

This challenging yet rewarding role requires a strong leader who can inspire and motivate teams to achieve excellent patient outcomes.

Requirements and Benefits
  • NMC or HCPC registration is essential.
  • A degree or equivalent experience is required.
  • Evidence of leadership ability, able to influence, involve and develop individuals and teams.

The salary for this position is estimated to be around £45,000 - £55,000 per annum, based on location and industry standards.
