Job Summary

We are seeking an experienced clinical pharmacist to join our team as a Pharmacist Lead for Anticoagulation Services. This is a challenging and rewarding role that will allow you to utilize your expertise in anticoagulation, thrombosis, and venous thromboembolism (VTE) to improve patient outcomes.

About the Role
  • This post holder will be responsible for the provision of a high-quality patient-focused clinical pharmacy service for patients within the General Surgery care areas alongside being the pharmacy lead for Anticoagulation, Thrombosis and VTE across the University Hospitals Plymouth (UHP) NHS Trust.
  • The successful candidate will act as team leader for the ward-based pharmacy team and as a team member of the Surgery pharmacy team, working closely with care group lead pharmacist & associate chief pharmacist for clinical services on the strategic development of clinical pharmacy services.

Key Responsibilities:

  • Develop and implement strategies to improve patient safety and outcomes in anticoagulation, thrombosis, and VTE.
  • Work closely with the anticoagulation team to provide specialist clinical pharmacy services and advice to the clinical specialties.
  • Monitor, review and report on medicines-related incidents within General Surgery and anticoagulation, thrombosis and VTE.
  • Lead the development and implementation of clinical pharmacy policy in conjunction with senior pharmacists within surgery and anticoagulation.
