Physician Team Manager

As a Community Health Leader, you will play a key role in leading our multidisciplinary team in providing dedicated support for high-risk patients and those recurrently admitted to hospital.
The programme aims to be coordinated, holistic, intensive, early, and focused, bringing together colleagues from medicine, nursing, pharmacy, social care, voluntary sector, therapy, and community teams.
You will provide clinical input and leadership to the team, defining a management plan that addresses the needs of the whole patient across physical, mental, and social care needs.
As Clinical Lead, you will also be responsible for line managing clinicians, ensuring high-quality clinical care, and participating in trust quality and safety work.
Main Responsibilities- Develop and implement strategies to improve patient outcomes and reduce hospital admissions.
- Lead the development and implementation of effective management plans that address the needs of high-risk patients.
- Collaborate with colleagues across different disciplines and settings to ensure effective communication and collaboration.
