Job Description:

As a Nurse Practitioner in Heart Rehabilitation, you will work closely with our multidisciplinary team to provide comprehensive care to patients with heart failure. Your primary responsibility will be to support the community patient clients, running outpatient clinics and relevant home visits as part of a team approach to care delivery and caseload management.

You will also be expected to provide clinical leadership, advice, support, and information to staff regarding all aspects of heart failure care. This includes developing a care pathway, improving discharge planning, and enhancing discharge information.

We offer a competitive salary package, including a base salary of £43,141 - £71,567 per annum, depending on experience, plus additional benefits such as generous pension scheme, annual leave, and training opportunities.
