Buckinghamshire Healthcare NHS Trust is seeking an enthusiastic and skilled Rehabilitation Therapist to join our Community Health Team. This is an excellent opportunity to work in a fast-paced environment, providing assessment, acute rehabilitation, and timely discharge for patients transferred from acute hospitals.

About the Job

As a member of our team, you will work in a multi-skilled setting, requiring experience in acute medical discharge planning, neurology, amputees, falls, and other pathologies. You will be supported by experienced therapists and clinical support workers, contributing to decision-making and patient care.

Your role will involve joint working with patients, physiotherapists, nurses, and community teams to achieve timely discharges. We offer flexible and agile working opportunities, alongside generous annual leave entitlement, pension, and access to NHS discount schemes.

The successful candidate will have a Diploma or Degree in Occupational Therapy and current registration with the Health Professions Council. Evidence of continuing professional development and reflective practice is essential. We offer a competitive salary, estimated to be around £40,000-£60,000 per annum, depending on experience.

Responsibilities
  • Assess and treat patients in acute rehabilitation settings
  • Develop and implement individualized rehabilitation plans
  • Collaborate with multidisciplinary teams to achieve optimal patient outcomes
  • Contribute to quality improvement initiatives and audit activities
