Occupation Therapist Stroke Bridging

We are looking for an enthusiastic and experienced neurological Occupational Therapist to join our Stroke Rehabilitation Bridging Service working in the community setting across Buckinghamshire. This role is part of a new service looking to improve the quality of the stroke rehab service by providing a seamless transition from early supported discharge to community rehabilitation. This role will suit someone who is passionate about providing high quality care and rehabilitation for stroke patients, working together with the MDT and in partnership with patients and families.
The post holder will be supported by the Stroke Rehab Bridging Service lead as well as working alongside other experienced Band 7 Therapists.
To be responsible for jointly developing the Stroke Rehabilitation Bridging Service Team team providing a service to stroke patients in their homes immediately following input from the Early Supported Discharge team.
To liaise closely with the ESD team to understand the patients ongoing rehabilitation needs and to prevent duplication of assessment.
To perform specialised assessment and treatment for patients with complex presentations and needs, and to provide expert advice, guidance and information to health and social care professionals, carers, relatives and others.
To undertake all aspects of clinical responsibilities as a skilled, autonomous and advanced practitioner.
To actively lead development of professional and service initiatives, and contribute to organisational objectives.
To supervise, teach and appraise Physiotherapy, Speech and language therapy, Occupational Therapy staff and students within the specialty.
