Job Overview

We are seeking a dedicated and compassionate Leaving Care Practitioner to join our team in Stockport. This is an excellent opportunity to make a meaningful contribution to the lives of young people aged 16+ who are leaving care.

About the Role

This role involves providing direct support to care leavers, helping them navigate the challenges of transitioning into adulthood. You will work collaboratively with young people, their families, and other agencies to ensure they receive the necessary support to reach their full potential.

Your main responsibilities will include:

  • Conducting regular visits and maintaining close relationships with care leavers.
  • Identifying packages of support tailored to each individual's needs.
  • Liaising with agencies to provide practical advice and assistance.

You will also be responsible for:

  • Promoting independence among care leavers.
  • Supporting daily skills development, such as financial management.
  • Collaborating with partner agencies to ensure comprehensive support.

To succeed in this role, you will need:

  • A UK Driving Licence and access to a vehicle with business insurance.
  • Experience working with care leavers or young people in care.
  • An Enhanced Child and Adult DBS Certificate.

Salary Range: £26,000 - £32,000 per annum.
