Leaving Care Social Worker

We are seeking an experienced Leaving Care Social Worker to provide critical support to young people transitioning out of care.
The estimated hourly salary for this role is £16.01, with a 36-hour working week.
Key Responsibilities:- Build relationships with young people referred to the project and engage them in the service.
- Conduct thorough support needs assessments with young people and develop personalized plans to achieve their future goals and objectives.
- Guide young people in accessing positive activities, education, training, and employment opportunities.
- Collaborate with statutory agencies and departments to ensure adherence to guidelines and procedures.
- Engage in direct work with young people and key stakeholders to foster trusting relationships.
- Empower young people by enhancing their self-esteem, confidence, knowledge, and resilience.
- Respond effectively to child protection and adult at risk concerns, following safeguarding policies and procedures.
- Liaise with other agencies on behalf of young people.
- Maintain trust and confidence with young people and link them to suitable services and support.
- Support young people in housing, health, education, training, and employment.
- Raise awareness about safe care and protection, CSE, and associated risks.
- Twice weekly payroll.
- Complimentary DBS and compliance services.
