Key Responsibilities

As a Nurse Practitioner in Community Childrens Nursing, you will be responsible for formulating individual care packages that facilitate the smooth transition from hospital to home. You will also participate in the out-of-hours telephone on-call system to support all children and families on the CCN countywide caseload.

The ideal candidate will have a Masters/BSc (HONS) community health degree with specialist practitioner qualification, or equivalent, and experience of working in the community setting and managing a nursing caseload. You will also have research and audit experience, with evidence of service quality improvement.

This role offers opportunities for professional development, training, and regular 1:1 support from the Team Leader. In return, we offer a competitive salary of £45,000 - £62,000 per annum, depending on experience, and a comprehensive benefits package.
