We are seeking an experienced children's nurse to join our Lincoln CCN Team in Lincolnshire. The ideal candidate will have a passion for community nursing and be committed to delivering high-quality patient care.

As a team member, you will formulate individual care packages for smooth transition from hospital to home, manage your caseload, coordinate services for early discharge, reduce hospital re-admissions, and provide support for children with acute healthcare needs or long-term conditions at home.

The service operates Monday to Friday, 9am to 5pm, with flexible working arrangements for 'before' and 'after' school visits. You will participate in an evening telephone on-call system and a 24/7 rota for children requiring end-of-life care.

Main Duties
  • Formulate individual care packages for transition from hospital to home
  • Manage caseload, coordinate services for early discharge
  • Reduce hospital re-admissions, provide support for children with acute healthcare needs or long-term conditions

Person SpecificationQualifications
  • NMC Registered Nurse (Child)
  • Teaching / mentoring qualification
  • Evidence of continued professional development
Previous Experience
  • Recent experience of working as a children's nurse
  • Experience of safeguarding children / child protection
Evidence of Particular: Knowledge, Skills and Aptitudes
  • Ability to evidence key values and behaviours in line with the Trust Framework
  • Excellent written and verbal communication skills
  • Knowledge of health & safety & risk management
