Key Responsibilities:

  • Assessment and Support: Conduct thorough assessments of young people's mental health needs, identifying areas for support and developing personalized care plans.
  • Collaboration and Communication: Work closely with multidisciplinary teams, including social workers, psychologists, and other healthcare professionals, to ensure seamless care coordination and effective communication.
  • Risk Management: Develop and implement risk management plans, adapting to changing circumstances and prioritizing the safety and well-being of young people.
  • Discharge Planning: Coordinate discharge planning, ensuring a smooth transition from hospital to community-based care, and providing ongoing support as needed.
  • Triage and Duty Support: Provide triage and duty support to the team, prioritizing caseloads and work to ensure timely interventions and effective outcomes.
  • Signposting and Referrals: Signpost young people to appropriate services and make safeguarding referrals when necessary, ensuring they receive the support they need to thrive.

Requirements:

  • Experience: Proven experience working with young people with mental health needs, including those with complex and high-risk presentations.
  • Skills: Excellent assessment and communication skills, with the ability to work collaboratively and adapt to changing circumstances.
  • Qualifications: Registered Mental Nurse (RMN) or equivalent qualification, with a strong understanding of CAMHS principles and practices.
