Key Responsibilities
  • Assess patients and develop personalized care plans to support their recovery and improve their quality of life.
  • Provide expert care and support to individuals in their own homes or in the community, working closely with them to achieve their goals.
  • Coordinate care by collaborating with other healthcare professionals and stakeholders to ensure seamless delivery of services.
  • Manage patient documents and maintain accurate records to ensure continuity of care.
  • Advocate for the people in your care, supporting and signposting them to access the resources they need to achieve their goals.

Requirements
  • Registered Mental Health Nurse Degree or equivalent.
  • Evidence of commitment to continuing professional development.
  • Understanding of Health and Safety policies and procedures.
  • Knowledge and practice of Mental Health Act 1983.
  • Experience of assessing, planning, implementing and reviewing care needs.
  • Experience of CPA Care Coordinator role.
  • Experience of care planning, risk assessing and safety planning.
