About the Role

We are seeking a highly skilled and motivated Clinical Pharmacist to join our Medicines Management team at Central London Community Healthcare Trust. As a key member of our team, you will play a vital role in providing a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings.

Key Responsibilities
  • Provide and develop a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals.
  • Support and guide Medicine Management Pharmacy Technicians in their roles.
  • Be responsible for individualised patient pharmaceutical care, ensuring clinical prioritisation on a day-to-day basis at ward level.
  • Provide a patient-focused pharmacy service to patients and staff, including medicines reconciliation and full clinical screening of prescriptions.
  • Facilitate changes to patients' own use of medicines to improve safety, adherence, and empower independent use of medicines as appropriate.
  • Support and implement efficient medicines management initiatives to reduce drug waste and drug spend, and to support patient flow and facilitate timely discharges.
  • Participate in ward rounds and multidisciplinary team meetings to ensure a multidisciplinary and integrated approach to medicines management.
Requirements
  • Vocational Masters Degree in Pharmacy.
  • Professional registration with the General Pharmaceutical Council (GPhC).
  • Postgraduate Clinical Pharmacy Certificate qualification or equivalent.
  • Hospital experience.
  • Experience of providing a Clinical Pharmacy Service with good medicines management initiatives.
  • Evidence of basic teaching and tutoring in a hospital setting.
  • Experience of working effectively within multi-disciplinary groups.
  • Experience of structured medication review.
