About the Role

We are seeking an experienced Home-Based Occupational Therapy Specialist to join our team at BCHC. As a Home-Based Occupational Therapy Specialist, you will be responsible for providing occupational therapy assessment and intervention services to patients in their own homes, promoting independence and developing individual plans of care relevant to patient needs.

Key Responsibilities

  • Providing occupational therapy assessment and intervention services to patients in their own homes, promoting independence and developing individual plans of care relevant to patient needs.
  • Working in partnership with patients, carers, and other health and social care agencies to achieve desired outcomes.
  • Developing and implementing treatment programmes at home and ensuring safe discharge or referral on to alternative services as appropriate.

Requirements

To be successful in this role, you will need to have a degree in Occupational Therapy, current registration with the Health and Care Professions Council, and extensive evidence of relevant post-registration training and CPD. You should also have experience of working in a multi-disciplinary team, relevant rehabilitation experience for a variety of patients in a community setting, and ability to plan and delegate daily workload.

Working Arrangements
