About the Role

We are seeking a highly motivated and experienced Care Team Leader to join our team at Skylark House Care Home in Horsham. As a Care Team Leader, you will be responsible for leading and supervising your shift, case managing residents, and liaising with families, healthcare professionals, and other stakeholders to ensure the best care plans are created and delivered.

Key Responsibilities
  • Leading and supervising your shift to ensure the highest standards of care are maintained
  • Case managing residents and their families to ensure their needs are met and their care plans are delivered
  • Liaising with healthcare professionals and other stakeholders to ensure the best possible care for residents
  • Administering and recording medication as required
  • Carrying out risk assessments to identify potential risks and take steps to mitigate them
  • Working closely with the leadership team to develop services and improve care
Requirements

To be successful in this role, you will need to have a strong understanding of care planning processes, be well-organised, IT literate, and adept at allocating and delegating work appropriately. You will also need to have a proven ability to lead and supervise others, be experienced in or happy to undertake training on administering medication, and have a natural ability to help make the lives of others more fulfilling.
