Job Description:

This is an exciting opportunity to join our team as a Community CAMHS Transition Worker. We are looking for an experienced and dedicated individual who shares our passion for providing high-quality care and support to young people transitioning from CAMHS into adult mental health services.

As a key member of our team, you will work closely with young people, their families, and other professionals to develop and implement personalized support plans. You will have the opportunity to work in a variety of settings, including community centers, schools, and clinics, to provide outreach and support services to our clients.

To be successful in this role, you will need to possess excellent communication and engagement skills, with the ability to work effectively with diverse groups. You should also have a strong commitment to promoting equality and diversity, as well as a positive attitude towards working with people with mental health problems.

The ideal candidate will have previous experience working in community mental health services or a similar role, with a strong understanding of risk assessment/management, maintaining professional boundaries, and advocating on behalf of young people. They will also have advanced verbal and written communication skills, with the ability to tailor their message to various audiences.
