Job Summary

Southern Health NHS Foundation Trust is seeking a skilled Learning Disability Charge Nurse to join its North & West Hampshire Community Learning Disability Service. As a key member of the multi-disciplinary health team, you will play a vital role in supporting the healthcare needs of individuals with learning disabilities in the North Hampshire Locality.

Main Responsibilities

The successful candidate will be responsible for managing their own caseload, conducting comprehensive assessments, and developing robust person-centered care plans. You will lead nursing care for service users referred to the team, utilizing the CPA process when indicated. Additionally, you will supervise, mentor, and teach registered and unregistered staff, as well as students.

About the Role

This is an exciting opportunity to contribute to leadership within the nursing team and wider multi-disciplinary team. You will need to demonstrate effective leadership and interpersonal skills, good communication and presentation skills, and be organized and self-motivated.

Requirements

The ideal candidate will possess a RNLD qualification, computer skills, mentorship course completion, and clinical supervision training.
