Learning Disability Charge Nurse

Southern Health NHS Foundation Trust is seeking a full-time Band 6 Learning Disability Charge Nurse to work in the North & West Hampshire Community Learning Disability Service.
This is an exciting opportunity to join a busy and well-established multi-disciplinary health team supporting the health care needs of people with a learning disability throughout the North Hampshire Locality.
The successful candidate will play a vital role as part of the community nurse team in a forward-thinking and innovative team, offering regular supervision and opportunities for professional development.
The role requires the ability to manage a caseload, complete assessments, and develop robust person-centered care plans. The successful candidate will lead nursing care for service users referred to the team using the CPA process when indicated.
The Community Charge Nurse will work within the learning disability team, undertaking comprehensive assessments, interpreting, planning, and implementing nursing treatment and care to service users who require an adapted learning disability service.
The role also requires the learning disability nurse to mentor, supervise, and support junior nurses, student nurses, and support workers.
Main duties of the job
The role of the Community Charge Nurse is to work within the learning disability team, undertaking comprehensive assessments, interpreting, planning, and implementing nursing treatment and care to service users who require an adapted learning disability service.
The role also requires the learning disability nurse to mentor, supervise, and support junior nurses, student nurses, and support workers.
