Support Services Coordinator

We are seeking a skilled and empathetic Support Services Coordinator to join our team at Cartrefi Cymru. This role involves coordinating and delivering support services to individuals with complex needs.
About the Role:
This part-time position requires exceptional organizational, communication, and problem-solving skills to ensure effective support delivery. The successful candidate will work closely with colleagues, families, and external professionals to provide high-quality services.
Key Responsibilities:
- Coordinate and deliver support plans, ensuring alignment with individual goals and needs
- Collaborate with colleagues, families, and external professionals to share information and coordinate services
- Maintain accurate records of individual progress, achievements, and challenges
- Develop and implement strategies to promote independence, self-advocacy, and community engagement
- Contribute to ongoing quality assurance and improvement initiatives
Requirements:
- Valid driver's license and access to a reliable vehicle
- Excellent communication, interpersonal, and problem-solving skills
- Ability to maintain confidentiality and adhere to data protection policies
- Basic computer skills and proficiency in record-keeping software
- Experience working with vulnerable adults or similar populations (desirable)
What We Offer:
