**About the Role:**

We are seeking a highly skilled and compassionate Social Worker to join our team at the State of Connecticut Executive Branch. This is an excellent opportunity to make a meaningful difference in the lives of vulnerable individuals and families.

Job Overview:

This position requires a Master's degree in social work or a closely related field, with experience in case management techniques and counseling to sustain or restore client functioning. Our ideal candidate will have respect for clients' rights to self-determination and understand the importance of appreciating diversity, including race, religion, ethnicity, sexual preference, gender identity, age, disability status, and individual values.

The successful candidate will be responsible for conducting home visits, evaluating clients' capacity to manage their own affairs, arranging supports such as homecare, public assistance, legal aid, housing, medical, and psychiatric examination, and communicating effectively with clients, family members, and other care systems.

Responsibilities:

  • Evaluate clients' needs and develop plans to address them.
  • Conduct home visits to assess client functioning and monitor risk.
  • Assess clients' capacity to manage their own affairs and protect themselves from financial exploitation.
  • Arrange for and assess the status of supports such as homecare, public assistance, legal aid, housing, medical, and psychiatric examination.
  • Communicate effectively with clients, family members, and other care systems.

Requirements:

A Master's degree in social work or a closely related field and one (1) year of experience in the self-directed use of case management techniques and counseling to sustain or restore client functioning. Alternatively, a Bachelor's degree in social work or a closely related field and two (2) years of experience in the self-directed use of case management techniques and counseling to sustain or restore client functioning.
