We are seeking a highly skilled Disability Services Professional to join our team at Entrust Social Care Ltd in providing comprehensive support to disabled children and their families.

Responsibilities:
- Support disabled children and their families within the Short Breaks/Early Help team.
- Conduct assessments, chair meetings, and engage in direct work with children and young people.
- Liaise with other professionals to ensure comprehensive support.
- Carry out Short Break Assessments and EHATs as required.
Requirements:
- DipSW or equivalent qualification.
- Registration with the HCPC.
- Current DBS clearance.
- Valid car driver's license.
